Please fill out this application if you are interested in umpiring baseball and or softball games for the South Lyon Junior League.  By submitting this application, applicants age 18 and older agree to allow the South Lyon Junior League (SLJL) and authorize the SLJL and its staff to perform a criminal history background check using the provided information. Once the background check has been completed, it is understood that this form will be destroyed for the stated season. The information requested is required by the background check system utilized by the Michigan State Police.
