Meal Delivery Form

New York Mills Public School will be providing breakfast and lunch for all children 18 and under, including pre-school children, beginning Wednesday, March 18, 2020. There will be one delivery each day. IN ORDER TO RECEIVE FOOD DELIVERIES, THE DISTRICT MUST RECEIVE CONSENT FROM FAMILIES TO DELIVER MEALS.

The school is working on the details to provide home delivery of meals directly to the children’s home. Delivery will be along bus routes during the morning hours. Please be patient as we work to put these systems into place.

PLEASE COMPLETE AND RETURN THIS FORM BY NOON, Tuesday, March 17th. Forms received after this time will not receive delivery until the day following receipt.

Grab and Go Meals
Alternatively, “Grab and Go Meals” can be picked up at the New York Mills Commons between the hours of 8:00 am and 11:00 am. Students cannot receive both the delivery and pick up a “Grab and Go Meal”.
