Selection Form and Payment Due Friday, January 14 by 11:59pm! This form will close at that time.

-Quarter III course offerings and sign up will be available online and can be found here: https://www.birmingham.k12.mi.us/Page/6952

-Choice Hour will run daily from 3:25-4:10pm. Students taking the bus or who plan to stay for an after school sport must be enrolled in choice hour (or they must leave and return for practice).

-Sign up is NOT first come first served. Anything submitted before the deadline is scheduled and processed together.

-Classes are offered IN PERSON

The announcement of selections and schedules for Quarter II will be available electronically via on Wednesday, November 3. Schedules will be posted at the school and distributed in homerooms on the first day of the quarter.

The number of selections offered for classes is determined by how many students attend Choice Hour each day.  To make the selection process operate smoothly, use the following guidelines:

• Three days of Choice Hour are required and offered at NO CHARGE.
• A 4th day is $55 for the quarter.
• A 5th day is $55 for the quarter. Students staying 5 days, owe $110 total.
• Payment Options: Cash, Check, or eFunds electronic payment. Payment is due by the sign up deadline.

Complete this form by the listed deadline.  Students who do not turn in a selection form with payment by the deadline will be considered late and will be scheduled AFTER all on time submissions are processed.

5 Choice Agreement
This form requires that 4 choices be made for each day. Students and families are expected to select 5 different choices for each day they plan to stay for choice hour. Students should expect to receive one of their 5 choices. If students fail to select 5 choices then they may be placed into a class that was not one of choices indicated.
