COVID-19 WAIVER AND RELEASE OF LIABILITY

By checking the AGREE box below you are acknowledging that an inherent risk of exposure to COVID-19 exists in any public place where people are present. By attending NJDBC dragon boat practice you voluntarily assume all risks related to exposure to COVID-19 and agree not to hold NJDBC, Ellen Law, Team Representatives, Captains, Lake Parsippany or any affiliates, liable for any illness or injury.

In checking this box, I acknowledge and represent that I have read and understand this release of liability and assumptioin of risk agreement, fully understand its terms, understand that I have given up substantial rights by signing it, and sign it freely and voluntarily without any inducement.
