PARENTAL/GUARDIAN CONSENT

1) First Baptist Church of Cramerton will provide transportation to the following locations: Church and Carowinds. Food and drink can be purchased from the vendors in the Park, and there will be time for a designated lunch break to be taken. The Church is not responsible for purchasing food/drink or other activities for your children at Carowinds. Parents/Guardians must indicate if there are any allergies and other medical information for their children.

2) Parents/Guardians must sign their children into the Carowinds Event by completing this registration form. Only the Parents/Guardians (or their listed emergency contact - in the case of an emergency) are able to check out their children from this event. If another adult(s) needs to pick up your child, please indicate that in the NOTES section.

3) Parents/Guardians agree to pick up their children at approximately 6:30pm, and no later than 7:00pm. Parents also agree to be ready to pick up their children in either the case of emergency or dismissal of the Child from the event. If the Parent/Guardian is unable to pick up their children after we attempt to contact, we will contact the Emergency Contact or other adults listed to pick up the child.

4) The Parent/Guardians agree that, if the Child is unable to be picked up in any case, the Church will arrange for the designated transportation volunteers to take the child to the listed Parent/Guardian address as a last resort. This will also be taken into consideration for allowing participation in future events/activities.

5) For this event, children must either be in the 6th Grade (or at least 10 years of age or older); and can not be over the age of 18 (unless they are still in the 12th Grade).

6) The daily rate of the amusement park will be subsidized by the Church so that it will cost $20.00 per person for entry to the park (unless the child has a Season Pass, then there will be no additional entry costs charged). Parents/Guardians are responsible for giving their children (or an assigned Chaperone) the monetary funds for purchasing all food/drink and other activities within the park. The suggested amount is $20.00 for each child to purchase food and drink. Please inform us in advance if you are in need of financial assistance.

7) Children must follow the rules set by the Church Staff and designated volunteers. Failure to follow rules may result in dismissal from the event at discretion of the Church Staff.
