PURPOSE: This form allows the Northwest Avalanche Lacrosse Club Board of Directors to follow a process for the review and selection of volunteers to fill coaching positions for the upcoming Northwest Avalanche season. Prior to any individual being selected for a position as a Head or Assistant Coach they must submit a completed application and submit a background check. The information collected will only be used as the basis for review and ultimate recommendation to the Northwest Avalanche Lacrosse Club Board of Directors for final approval of appointment of the applicant to participate in a coaching position. We do not anticipate challenges, however we are committed to maintaining a quality and safe environment for all participants. All results will remain confidential.
INSTRUCTIONS: This application must be returned no later than January 21st. Applications received after this date will not be guaranteed consideration for a coaching position for the upcoming season. The Northwest Avalanche Lacrosse Club Board of Directors will consider and appoint candidates until all positions are filled.

**Northwest Avalanche Lacrosse is an organization that is run strictly by volunteers. These volunteers receive
no monetary compensation or benefits for their services. Northwest Avalanche Lacrosse Club promotes
health, fitness, teamwork and academic recognition for all of its young athletes.** All coaches or
representatives for the Northwest Avalanche Lacrosse Club are expected to maintain the highest standards in
sportsmanship and to display these standards, by example, for the benefit of their team.
