Thank you for coming in to talk with a volunteer at the Peer Support Centre.

We're always trying to make our service better, which is why we've asked you to tell us about your experience.

All the information given in this form will remain confidential. This means that no personal information will be taken, no volunteers will have access to the information, and only the Peer Support Centre admin staff will have access to the answers. The survey data will be collated so that overall impressions will be the focus, and so that we don't know who submitted the feedback.

Whether you choose to complete the form or not, will not affect your ability to access the centre in the future. Anyone can use the Peer Support Centre if a new issue arises, or their situation changes. This survey will only take about 5 minutes to complete. No questions are mandatory, so feel free to skip any questions.
