Complete parts 1, 2 and 3 completely, using the Google Form.
For part 2: All you should do is check any of the sports you have participated in at Middle Creek HS and the role you had on the team (There are two sections, one for athletic participation, another for manager or trainer). Do not list JV and varsity separately. EXAMPLE: If you played JV & Varsity basketball you only need to list basketball once on part 2. Please list a summary of any awards received for verification by the Athletic Director and Coaching Staff. Do not give this to your coach to complete, as it will be completed by him or her at a later date using their own form!
Make sure to give as much detail as possible when you complete part 3 (community service), as long as it will fit in the space provided in the Google form.
Please answer all 4 questions asked on this application when you complete part 4 (essay)! Part 4 (essay) must be typed on a separate sheet of paper. ***Please do not include your name anywhere but the top of the page for identification purposes.
Return Essay Section of the application to Student Services/Guidance Office by Monday, May 6, 2024. Google Form for Sections 1, 2, & 3 MUST be completed by ABOVE DUE DATE as well!
The athletic director and coaches will verify sections one and two after you have turned in your application. The athletic director will get your completed application turned in to the Stampede Club for you.
