Use this form to add contact info for a BOARD REP, PARENT REP, OFFICIALS COORDINATOR, COMPUTER REP, COACH, or CHAMPS COORDINATOR for your team. For each contact person, you must submit a new form. If you have multiple coaches, submit a new form for each person. Please use each submission for only one team position (for example, if you are adding a Board Rep, please only list the previous Board Rep’s contact info to be deleted--don’t add a Board Rep but delete a Parent Rep all on the same form).
