Custom Candle & Spray Request Form
This form is to be filled out for purchasing a custom blend.  You can purchase up to 3 custom blends during this time.

The steps to create a custom candle include:
1. Purchase your slot in line on our website here:

2. Fill out this form for EACH custom item you want to purchase:

The slot you purchase is based on your item size and the minimum components. If you fill out this form and add on upgrades, your invoice will be edited and you will receive a notification that you have an additional balance. All additional balances MUST BE PAID within 48 hours or your request will be refunded so the next person waiting in line can purchase a custom slot. There are NO exceptions.

Custom blends will take 3-4 weeks to process.

If you do not see an oil option on this list, it is not available during this round. I will not be making any 'off the book' custom blends.
