Dear Parents/Guardians,

Please fill out this form FOR EACH CHILD who will be attending Q300 for school year 2022-2023

Please review eligibility guidelines to determine if your K - 6th grade child is eligible for OPT yellow bus or MetroCard service.  Please note, there is no OPT bus service for grades 7 and 8.

Both services are free to eligible families and are provided through The Office of Pupil Transportation (OPT).
