St. Francis Xavier Life Teen Consent 2020/2021
Below please complete the forms giving permission for your teen to participate in our Life Teen activities for the time period August 1, 2020, to July 31, 2021.  This includes Sunday Life Nights, Teen Tuesday, Theology Thursday, Life Teen Band, any on-campus retreats, retreats held at St. Leonard Retreat Center, Steubenville, Cove Crest, LTLC conference and any field trips or other offsite conferences not listed above.  This also includes any virtual meetings via Zoom held throughout the year.  Please note that some offsite conferences will have their own forms that will be emailed if your teen is a participant.

Upon reception of your registration, you will be emailed a confirmation that your information has been received.
