“I understand that I may incur an injury while playing softball and/or participating in softball-related activities and am willingly participating in such activities, including but not limited to Play Days, clinics, socials, practices, League games, scrimmages, tournaments and any other San Jose Women’s Softball League (SJWSL) or NAGAAA/ASANA related activity such as travel, meetings and fundraisers AT MY OWN RISK. By signing this form, I release all other participants, associates, cities, host venues, sponsors, volunteers, organizers and property owners from any liability for injuries, harm, loss, or embarrassment that I may suffer as a result of my participation.

I am at least 18 years of age.
I agree to abide by the League’s Code which includes the S.A.N.C.R.A. Player Code of Conduct.

I understand that this League, known as the San Jose Women’s Softball League (which may be referred to as “SJWSL” or the “League”) is a player-run, volunteer-run, nonprofit organization and therefore, by joining a team in this League, I understand that I will be encouraged to help with some of the tasks it takes to keep the League going, such as the staffing of Play Days and the San Jose Pride player-recruitment booth.

I am hereby informed that I am also welcome to participate with a women’s team in our “sister” league, the San Francisco Gay Softball League, but that if I do so, then I must inform the managers of both teams or be subject to a $100 fine if one of the team’s league or tournament rosters becomes compromised at a regional or national level due to my failure to inform both managers, verbally and in writing, of my dual participation.

I am also hereby informed that the League is required to provide ASANA with an annual list of its spring session rosters showing just players’ first and last names (NO personal information such as address, phone, email, etc.) in order for all its teams and players to be eligible to participate in ASANA tournaments.
