*IF YOU HAVE TESTED POSITIVE FOR COVID OR HAVE COME INTO CONTACT WITH SOMEONE WITH COVID, YOU MUST HAVE A NEGATIVE COVID TEST 10 DAYS AFTER SYMPTOMS END or 10 DAYS AFTER CONTACT IF NON-SYMPTOMATIC, PRIOR TO RETURNING TO THE SALON*

I have not travelled outside of my immediate daily routine for the past ten days. *
I do not have a cough, fever, chills, shortness of breath, or loss of taste or smell. *
If I begin to show symptoms of COVID-19 within the next ten days, I will contact BG Salon. *
I will follow all salon rules to keep myself, the BG staff and those around me safe. *
