Permission is hereby granted to the Director of Bands, Athletic Director or Athletic Trainer at Union Academy Charter School to proceed with ANY needed medical treatment, ambulance notification, and/or x‐ray for the below named student. In the event of serious illness or injury, need of surgery, or accidental occurrences I understand that an attempt will be made by the director of bands, coaching or medical staff to contact me by phone. If unsuccessful, needed emergency treatment may be given as necessary for the best interest of the student and a copy of this permission form will be furnished to the doctor in charge. I also grant permission for the treating physician to release information to the director of bands, athletic trainer and/or health related information needed to care for my child with physicians, coaches, other healthcare providers, etc. throughout the school year. *

WE NEED VOLUNTEERS!!! It takes ALL of our band families to create a successful Marching Band Program.  The following link will take you to a form with volunteer opportunities available for marching band and beyond. Please check next to any positions you would like to learn more about. EVERY family is asked to work at least one shift at band camp and participate in at least one fundraiser.
