This form is for individuals or companies that have commissioned art from MiddleVerse.
All custom MiddleVerse artwork is strictly based on published MiddleVerse products.
We do not "insert" real or fictional individuals, including you, other than MiddleVerse into any artwork.

You must have already paid for your purchase to complete this form.

This form is required to be filled out completely in order to process your commission.
Artwork is usually delivered 7 days after completing the form.
If there are any issues or questions regarding your form answers we will reach out via email to you.
Your commission price includes a total of three (3) redesigns to ensure your satisfaction.
If you require more than that you can purchase additional redesigns for $20 per redesign.

If no request is made for a redesign within seven (7) days of the last sent art then the order will be considered complete and final. Redesign request(s) must be emailed to same email you received the artwork from.
If you purchased with ownership rights to the commission art you will receive a statement asserting your ownership within eight (8) days of the final art delivery.
