FPCR Nursery Reservations

Please fill out the following form to reserve a spot for your child in the nursery. The nursery is open to children aged 0-5 (Kindergarten). Please note that your reservation is not confirmed until you receive an email from Gilli Smith on Saturday morning.

I understand that my child's nursery reservation is not confirmed until I receive an email from Gilli Smith.

If my child is aged 2 or above, I agree to bring a mask for them to wear at all times while in the nursery.

I agree to screen my child for COVID19 symptoms before bringing them to FPCR (fever, chills, cough, shortness of breath, fatigue, muscle or body aches, headache, loss of taste or smell, sore throat, congestion or runny nose, nausea, vomiting, or diarrhea).

If my child is exhibiting any COVID19 symptoms on Sunday morning, I agree to keep my child home from the nursery.
