Select the shifts you would like to work below by clicking the box under the word "Interested" for each shift.

Add any notes such as carpooling, potential late arrivals, etc. in the optional note section at the very bottom of the form.

Click "Submit" when you have completed your request.

You will receive a receipt via email that shows each shift that you requested. PLEASE REMEMBER that Scheduling will email you with your CONFIRMED schedule and that your requests are only requests until your CONFIRMED schedule is returned by Scheduling.

Once processed, you will receive an email from your scheduler with your confirmed schedule indicating Confirmed or Added to Avail for each shift.
