NAM2021 code of conduct violation reporting form
Please use this form to report any violations of the code of conduct (https://nam2021.org/bath/code-of-conduct) you were subject to witnessed. The code of conduct applies to all meeting events and social events, including on the meeting platform (Hopin), Zoom, YouTube, on the NAM2021 Slack space and in the Gather.town space.

You are welcome to remain anonymous if you wish. However, the LOC is limited in the actions it can take following anonymous reports. Any information you provide will be treated confidentially. We will retain any reports for the duration of the conference.

The form will be monitored regularly and any information received will be accessible only to the following people: Robert Massey (RAS, rmassey@ras.ac.uk), Stijn Wuyts (Bath LOC, s.wuyts@bath.ac.uk), Hendrik Van Eerten (Bath LOC, hjve20@bath.ac.uk), Carole Mundell (Bath, SOC co-chair, cgm37@bath.ac.uk) and Cathryn Mitchell (Bath, SOC co-chair, eescnm@bath.ac.uk). The only data that is collected besides the information you provide is the timestamp of the submission.

If you would prefer your report not to be shared with all members of this group you are welcome to email any of the listed people individually.
