Volunteer Application Fall Semester 2021
The volunteer program is open to Sparta Library cardholders in Grades 7-12 in the Fall of 2021. Applications will be accepted through September 30th. Once we have reviewed your application, you will receive an email with more detailed task lists and a link to Sign Up Genius. Please make sure all sections of the application are completed prior to submitting your application, including an email address that is checked regularly.

My child has my permission to participate in this program and I have reviewed all the questions for accuracy. I grant the Sparta Library permission to share images/video of my child. When applicable, I give permission for my child's name and grade to be used (for ex., newspaper). All volunteers must also submit a completed Minor Volunteer Hold-Harmless Waiver signed by a parent or guardian before his/her/their first volunteer session.   *
