Deer Park High School Project Grad Commitment Contract
This form is to register your senior for the all night lock in known as Project Graduation.  For the last 30+ years Deer Park High School has provided a safe and fun environment for graduating seniors to gather together for one last celebration.  By signing below, my parent/guardian and I understand and agree to the following conditions of participation for DPHS Project Graduation.  Project Graduation will occur on Thursday, May 27th from 11 pm to 6 am at Deer Park South Campus.

This form must be completed, signed  by student/guest and their parent/guardian.  You may fill out the form at any time before the event or upon arrival.  Students may each bring one guest.  Guests must be under the age of 21.  All guests and their parent/guardian must also complete a contract.  The cost of admission for the senior is $21.00 (pre-paid) or $25.00 the day of the event.  All guests are $25.00.  All students and guests are required to present a photo I.D. for admission to the event.  No Exceptions. Guests are not eligible for prizes, gifts, and T-shirts. *

In case of emergency students will only be released to their parent/guardian.  If released, students will not be allowed to re-enter.  Students must attend the full duration of the event to be eligible for prize drawings and gifts.  In case of illness, parents will be notified.  If the student tries to leave before the end of the event, the parents will be notified at the number provided. *

Students and guests will come to the event free of any drugs, alcohol, or tobacco products and will not be admitted if it is suspected that the student has already consumed drugs or alcohol.  The use of any of these products during the event will result in immediate removal and a phone call will be made to the student's parent/guardian for pick-up and transportation.  Local police officers will be on site to deal with inappropriate behavior or violation of the law. *
Students will respect the event site and do no malicious damage.  Appropriate clothing must be worn.  Shirts and shoes are required, Purses, string bags or backpacks are strongly discouraged. *
Transportation to and from DPHS is the responsibility of each student or parent/guardian. *

As a parent my initials below state that I completely agree to all the conditions above and understand everything I have read.  I also give permission for my senior/guest to participate in Project Graduation on May 27-28, 2021.  U understand that if he/she violates any of the terms or conditions, I will be contacted if he/she is asked to leave the event.   I hereby release Deer Park High School Project Graduation, DPISD, their respective affiliates, subsidiaries, volunteers, and agents harmless from any and all claims, damages, or injuries which may arise from my child's use or presence on such premises and activities thereon. Payment can be made in multiple ways, please choose the method in which you will pay.  I will make sure when I pay my seniors first and last name will be added to the memo along with the word Registration (Ex: Karona Virus, Registration) *
