At my request to participate in the events of the Ullswater Yacht Club, I agree that I will not for myself or the above named, hold the Club, its Officers, members or assistants liable for any injury or damage or loss suffered by the above named while engaged in club activities either on Club premises or elsewhere, or activities for which the club is or may be responsible and will indemnify the club, its Officers, members and assistants against all actions, claims or demands which might arise.

Parents and Guardians are warned that the club is only able to provide safety facilities during the hours of the clubs Sailing Programme. Parents and Guardians have sole responsibility for their children and wards and must appreciate that the club cannot be expected to exercise supervision or control.

Club Officers, Race Officers, Safety Boat Drivers and Crew or any other members of the sailing management team do not accept responsibility for any loss, damage, or injury suffered by persons and / or their property arising out of, or during the course of their activities whilst sailing / training / coaching and / or instructing unless such injury loss or damage was caused by or resulted from negligence or deliberate act.

• I accept responsibility for his / her conduct while sailing on Ullswater and whilst on the club premises.
• I understand that sailing has its attendant risks and that the club can not accept responsibility for Youths.
• I undertake to ensure that he / she will be suitably clothed while on club premises and whilst sailing.
• I understand that my child may be photographed by the club or its agents and the photographs or video used for promotional activity. If you do not wish your child to be photographed for whatever reason please tick the box at the bottom of this form.
• I accept responsibility for the seaworthiness of his / her boat and for its “adequate” insurance against third party claims. This must be at least £2 million.
• I understand the decision to allow the above named to participate in any event or training activity is my sole responsibility.
• I declare that the above named can swim 25m with a buoyancy aid.
• I undertake for those under 14 yrs, to ensure that a Parent or Guardian will counter sign the on / off sheet before each race.
• I declare that I have disclosed any medical problems that might affect the above named during the course of the Club activities. I consent to any emergency medical treatment necessary during the course of any event.
