Please complete this form for you and your guests and pay the funds owing into the bank account of the NZAS Canterbury Branch number 03 0802 0095005 000. Please reference MWD and your last name. - please book and pay by 30 June 2022, as the Chef likes to receive our orders by 1 July.

If you wish to book for more than five people, please complete this form again for the additional guests.

The venue is the Wigram Hotel, 14 Henry Wigram Dr Christchurch 8042. Join us at the Harvard Bar from 6pm and proceed to the Ballroom from 6.30pm, to be seated by 7pm.

Costs are $55 per person, which includes bread, main course, dessert and coffee or tea.

When booking, please make menu selections for you and your guests.

Main courses choices are:
- Merino Lamb Shank,
- Vol Au Vent with Chicken,
- Vegan Vol Au Vent (suitable for vegan, dairy free and vegetarian diets),
- Specific dietary requirements.

Dessert choices are:
- Sticky Date Pudding,
- Pavlova,
- Specific dietary requirements.

Regarding COVID-19, we are still operating under 'orange' and encourage you to wear a mask where practical.
