CIS Elementary Mobile Device Agreement
The iFive Technology Initiative in District Five provides students the opportunity to use a mobile device to engage in anytime, anywhere learning. Students can use these tools to research information, create projects, engage in class presentations, upload and download digital material and actively engage in communication and collaboration on their academic work.

Please visit the iFive Instructional Technology Initiative web page (see link below) where you will find the District Issued Mobile Device Guide. The guide describes the terms and responsibilities of accepting a district issued device. Please read it thoroughly before selecting yes or no at the bottom of this page.

To be issued a device, parents must also agree to the internet Acceptable Use Agreement that allows the student to utilize the district network. This was done in Registration Gateway.

Teachers will be providing live virtual instruction. Do you give permission for your child to be in this recording?  If not, we will have your child turn their video off during the class.  They will be able to see the teacher and others, but their face will not be visible. *
By signing/typing your name below, you are giving permission for your child to use a District Issued Mobile Device at home for educational purposes and you acknowledge and accept the agreements outlined in the Mobile Device Guide. *
