Please complete and submit this document if you would like us to provide you with a box of Numbered Offertory Envelopes for this year. A yearly tax receipt will be provided for donations received using these numbered envelopes.

Once we receive your request, an envelope box will be left on the counter at the back of the church with your name. Please pick it up at your earliest convenience and we thank you for supporting the parish
