This is the form for pet owners seeking assistance with veterinary care. We can only assist with care we provide at our facilities, no specialty medicine.  Healthy pets needed any veterinary assistance will be require to be spayed/neutered and microchipped. 

BEFORE FILLING OUT THIS FORM PLEASE REVIEW THE FOLLOWING AS WE OFFER MANY LOW COST VETERINARY OPTIONS:
Low pricing for spay/neuter surgery (lowest pricing ~$75/dog and ~$60/cat) Healthy pets needed any veterinary assistance will be require to be spayed/neutered. Vaccinations, nail trims, heartworm testing, and microchipping can be done at the same time as surgery.

Low cost medical care, please call or email to inquire about pricing. MUCH more affordable than regular veterinary care if you meet qualifications to use services.

Low cost vaccination clinics, first 125 pets, all are welcome. Held Saturdays 9AM-12PM.

FREE microchipping can be provided any day of the week!

Pet Food pantries are held on the 2nd Friday of the month 12-2PM at 21800 Central Ave. Matteson, IL 60443
