We are excited that you have chose to tryout for Nation Ford High School. Please read the information carefully and complete. Understand that the completion of this form along with your signature ensures that you fully understand the commitment and expectations of this season. You understand that failure to follow instructions or adhere to the rules will result in and up to dismissal from the squad. Also remember that if for any reason you can not meet the expectations, your spot on the team could be taken. This form must be filled out before first day of tryouts. Failure to have completed form will result in not being able to tryout.
