Private Lessons are strictly between the coaches, players and parents. YJ-MA Director will try to place players with the right coach for a lesson based on the information provided in the Registation Form. Parents and Coaches both understand last minute changes or cancelations can happen because of Covid.  Parents are responsible for paying Coaches directly. YJ-MA does NOT collect any money or provide payment methods. Coaches and Parents are RESPONSIBLE for setting a Date, Time and Location for lesson without YJ-MA help or involvement. Parent or guardian filling this form out MUST check the box below.   *

WAIVER and RELEASE OF LIABILITY: Taking Private Lesson is completely between the Coaches & Parents who agree to the Date, Time, Location for the Lesson. Parents have ALL the resposibilities to contact the coach(s)  and organize the lesson and pay coaches. The infomation provided below will only provide a recommendation to certain coaches. Parents, are fully aware that girls lacrosse is a physically active sport. Inasmuch, there are inhereted risks, including physical injury involved in playing or training. Parents release YJ-MA Coaches from liability resulting from any personal injury sustained  as a result of participation during any training/lessons. *

I agree to pay the coaches according to the pay scale below or based on the individual coaches own pay scale: Specialized Coaches for YJ-MA Players is $50 per hour and $75 per hour for non-members. Youth Coaches (HS or College Players) $25 for YJ-MA Players and $40 for non-members. Small Groups and other options are based on individual coaches decisions. *
