I understand that participation in this program is completely voluntary. I understand that workshop facilitators have been provided with detailed guidelines and the Parsec Youth Network makes efforts to ensure the safety of everyone involved throughout the process by regularly checking in and providing feedback forms. I accept and take full responsibility of all risks, loss, injury or damage that may be caused by an individual's negligent or intentional acts or omissions committed prior to, during, or after participating in the Parsec Youth Network summer workshop program. I understand that as a participant, I may have access to confidential information including such as the name and knowledge of personal life events of the workshop facilitators or other participants. I agree that I will not disclose this information to anyone participating in workshops with the Parsec Youth Network, unless required or permitted by law or at the request of the Parsec Youth Network. I, the participant, am at least eighteen (18) years of age OR I am the legal parent/guardian of the child who is under eighteen (18) years old and will be participating in workshops with the Parsec Youth Network. I agree to this waiver voluntarily with full understanding and knowledge of its significance. If I disagree with the waiver, I will cancel my registration to the current and future workshops by the Parsec Youth Network. *
