This form should be completed by anyone requesting use of the Unitarian Universalist Church of Chattanooga facilities for anything other than a UUCC sponsored event. Before you complete this form, please contact the church Administrator to learn more about our rental procedures, fees, and to determine if the facility is available at the date/time you are requesting. After you complete this form, someone from the UUCC will contact you regarding next steps.
