Privacy & Confidentiality: The information you provide will be recorded anonymously and solely be used for the purpose of this research and the recommendations developed from the results. Your responses will be collected anonymously and will not be tied back to you as an individual or your personally identifiable information. Your individual response will not be reported to anyone outside of NURILENS or its constituents.

Risks and Benefits: The benefits of taking part in this study include (1) impacting how eye health researchers and practitioners understand and enhance eye care services and resources and (2) contributing to under explored areas in eye health research.

We do not anticipate that you will experience any distress during this questionnaire.
