Sun Prairie Community Schools is excited to soon be able to offer nutritious meals and snacks for children over the weekends and during extended breaks, free of charge. The program is intended to help those children whose families find it difficult to have enough food for the entire month, but all children are eligible for this program.

To assist in our planning efforts, in order to best serve our community, please complete the short form below if you are interested in participating in this program.

Only one form is needed for ALL the children in your family, but include information for each child in the form below.

This information is private - by providing your contact information, you will receive information on where/when food pick up will be available and we will use the number of students for meal counts.

This program is possible due to funding from the City of Sun Prairie and is supported in partnership by the Sun Prairie Area School District, Sun Prairie Emergency Food Pantry, Dane County Public Health, Second Harvest, Sun Prairie YMCA and the Boys & Girls Club.
