Meal Request Form
Easton Valley Schools will offer a lunch at the normal cost of a school lunch during the school closure.  MEALS WILL BE FREE TO ALL STUDENTS ENROLLED AT EASTON VALLEY.  Please complete this form by 9:00 am on the day you are requesting a meal.  School staff will be preparing the meals and CURB SIDE pick up will be utilized for all locations and meals will be available for pick up on MONDAY, WEDNESDAY AND FRIDAY.  We will be delivering two meals on Monday and Wednesday and one on Friday to reduce the potential exposure to any staff or students.  

Pick up times are listed below:
- Preston 11:30-12:00
- Miles - 11:30-12:00
- Sabula - 11:30-12:00
- Spragueville - 11:30-12:00
- Green Island 11:30-12:00

When completing the form, please indicate which town location you will be using to pick up your meal(s).
