Use this form to register your middle school student for our youth ministry program - Catholic Stewardship Initiative (CSI). This ministry is open to all students in Grades 6 - 8. The information submitted will be used to keep parents informed of upcoming events and the work of the CSI Program. Email will serve as our primary means of communication. We will use email to send periodic reminders about upcoming events and program updates. Please be sure to provide the email address of the parent or guardian as no direct email communication is permitted with youth without written permission of a parent.
