WVUSD Child Care: 2021-2022 School Year Pre-Registration Form
IMPORTANT NOTICE:  

After submitting this pre-registration form, please wait for an email from the WVUSD Child Care office, which will include instructions and documents required to complete your child's enrollment.  All completed documents along with registration fee (checks only) must be submitted to the Child Care Office via our mail slot located on the side door of the Child Care office building within 5 calendar days of your pre-registration submission or your spot will be released.

Please note that you must submit one pre-registration form per child.

In order to comply with reopening health guidelines, we will only be able to enroll a very limited number of students. Space is first-come first-serve and a waiting list will be established once we reach full enrollment. If enrollment is full, we encourage you to add your child to the waiting list. If we are able to safely increase our capacity, we will do so by filling spots from the waiting list.

Registration Fee, Disclosure & Acknowledgement
A registration fee of $50.00 ($35.00 during the early bird period of May 17-May 27) per child for the 2021-2022 School Year will be collected at time of registration. The registration fee is non-refundable and non-transferrable.

To complete enrollment, all requested documents (to be provided by the childcare office) and registration fee must be submitted to the Child Care office, via mail slot, no later than 5 calendar days from the pre-registration submission. If the additional documents and the upfront registration fee are not received in the office by its due date, the SY 21/22 pre-registration will be null and void.
