Upon signature of this sign-up form, I agree to become a member of the Luxembourg Lacrosse Club a.s.b.l. (“LLC”). 

I hereby acknowledge and agree to the following conditions:

• Membership Fees:

• The membership becomes active once the first membership fee payment is made, except in case the member is explicitly exempt from the obligation by the club committee.

• The annual membership fee, set annually at the AGM, becomes due as of September each year. Members joining within the ongoing year shall pay a pro-rata of the full amount of the membership fee, being 1/12 per remaining full month until the following September.

• The first membership fee is due within 30 days of the date of signature of this membership agreement and that upon failure to pay the first or annual membership fee, I automatically lose my membership status and its related rights and obligations.

• The annual membership fee is paid onto LLC’s bank account.

• Insurance:

• The insurance conditions as outlined in Annex I.

• The LLC does not have any insurance policy for non-members and all risks of non-members participating in practices of the LLC are at the non-member's own responsibility.

• Only members with a competitive license and valid "Medico-Sportif" are covered by the LLC's insurance policies when participate in league games, tournaments or other competitive events as outlined in Annex I.

• Members with a competitive license or social license are covered by the LLC's insurance policies when particating in club practices or other non-competive events as outlined in Annex I.

• General:

• I accept that my personal data is collected for administrative purposes and that the association may contact me to inform me about its activities. 

• I declare having read and understood the statutes of the LLC and to respect all the obligations associated to the membership that are stated in the statutes and team rules (if available).

• I declare having read and understand the contents of this membership form, as well as Annex I.

The L.L.C. adheres to the following insurances schemes:

• The group insurance scheme for individual accidents, as well as a civil liability, as provided by the Ministry of Sport of the Grand-Duchy of Luxembourg and the Caisse de secours mutuels des sportifs (CSMS) for licensed players.  

• The group civil liability insurance scheme of the “Luxembourg Field Sports Federation” for non-sport related activities, of which the “Luxembourg Lacrosse Federation” and its related clubs are affiliated to.

The L.L.C. is not adhered to any other private insurance scheme.

Competition Licence

Pursuant to the grand-ducal regulation of 23 December 2016 concerning the mandatory medical examination for licensed members of authorized sports federation (the “Grand-Ducal Regulation”), any competitive athlete is subject to a periodical medical examination (the “Medico-Sportif”). The Medico-Sportif is mandatory for all athletes that intend to obtain a competition license, which will be issued by the Luxembourg Lacrosse Federation a.s.b.l. (the “Competition License”), and is required to competitively participate in any sporting activity. 

Failure to obtain and maintain a valid competition license results in the absence of insurance coverage during all competitive activities of the LLC, such as league games, tournaments or national team camps & events. LLC or Luxembourg Lacrosse Federation a.s.b.l. cannot be held liable for failure to obtain the License.

A valid membership is required to benefit from a Competition License.

Social Licence

A member with valid membership but invalid Medico-Sportif is granted a non-competition license known as “Social License”. Social License holders are excluded from attending competitive events.

A competition license is not mandatory to attend regular club practices, non-sport club & federation events or friendly games. 

General:

Due to the inherent risk of injury, any member of the LLC is further encouraged to obtain additional insurance coverage that will cover additional risks on top of those covered by the above-mentioned group insurance policies.  GDPR:

We are each year asked for information on our members to provide the Comité Olympique et Sportif Luxembourgeois (COSL) data on our demographics. Further, the insurance provided through COSL by way of the Caisse de secours mutuels des sportifs (CSMS) requires that we list our current active players, both competitive (licensed) and non-competitive (social). These data include: Name, Date of Birth, Nationality, gender and validity of a Medical License from CNS (Médico Sportif). All personal data collected is treated according to EU RGDP (GDPR) regulations. The registry of data treatments is available on request.

Your privacy is very important to us. We seek only to gather the basic information necessary to comply with the guidelines of COSL and the Ministry of Sport. Only Board members with the need to access your information can do so. This data is stored on a secure server and shall be deleted two years after you leave the federation as member.

Furthermore, as a member of the Federation, your image at events (sporting and social) may be used as part of our social media campaign and marketing promotions. By paying a member fee to your club, you agree to allow the club anmd federation the use of any images described above. If you wish to OPT OUT, please notify a committee member of the club or federation in writing.

Your cooperation is greatly appreciated. These data provide the Ministry and STATEC important information on athletes in the Grand Duchy as well as guide any financial assistance provided by the government. If you have any questions or wish further clarification, please don’t hesitate to contact us!
