This form is to be completed by parents / carers who want their child(ren) to attend URS Holiday club. A seperate form must be completed per child.
The deadline for all forms to be completed is the 29th November 2019.
Places will be allocated on a first come first serve basis, and an email confirming your childs place will be sent to the email address provided.
