In order to fulfil our obligations under Health and Safety law, I would be grateful if you could complete the form below.

I confirm that I hold a full driving licence suitable for the type of vehicle.

I have a current valid insurance policy for the vehicle in which I intend to carry children.

The vehicle is in a roadworthy condition and I have a current MOT certificate (if more than 3 years old).

Each child will have a recognised seat with a seatbelt and the vehicle cannot carry more than 8 passengers.

No child under 135cm will be transported without a booster seat (this may need to be provided by the child's parent).

I understand I am not covered under the above establishment's insurance for the use of my vehicle.
