EMS 5 Star Craft Fair Application 2023
Hello and thank you for taking the time to complete the following application for Eisenhower's Annual 5 Star Craft Fair!  We can't wait to see everyone again on December 9th, 2023. We are anticipating a a sellout of vendors. So please apply ASAP! We will not be allowing any drugs, weapons, or inappropriate material to be sold. 

Please note that you will be notified of acceptance into our fair by email, no later than October 25th. At that point, you will receive instructions on how to pay the booth fee. Booth fees are $75.

Please note only one type of each Direct Sales Company will be offered a booth.  Acceptance is on a first come first serve basis.  It is also important to note your application is not considered complete until payment has been received by the EMS PTA.  If another Vendor of the same Direct Sales Company has already been accepted you will be placed on a waiting list in the event changes occur.
