RUISSEAU CREOLE - ROLLER SKATING - AUTUMN PACKAGE (JUNE ONLY)

Dear Parents and Friendly Pupils,

We are welcoming you to register for Roller Skating Lessons at RUISSEAU CREOLE for the month of June 2019 on Saturday's Mornings, starting as from the 8th of June 2019 from 9:30 am to 10:45 am. (Other time frames on request only 08:00 am to 09:15 am and 11:00 am to 12:15 pm)

The area allocated: Parking closed to MCB at the entrance.

Dates:
Saturdays (3 Sessions + Replacement):  08/06, 15/06, 22/06 and 29/06 (as a Replacement Lesson) at MUR 1 000 for the Global Package or MUR 350 per lesson.


Who can register?
Anyone from 4 years and above

Age Categories (according to Sport's Calendar Year 2019 from January 1st to December 31st):
4-6 ;
7-9;
10-12,

Other Age Categories, on request, and pupils not at school will be able to register for lessons the soonest in your region.
13-15,
16+ ;
26+;
36+

Levels (according to monitor evaluation):
Beginners
Intermediate (Leisure 1 or Leisure 2)
Promising
Advanced

The Discipline proposed is Inline Skating (Main Disciplines: Slalom / Inline Hockey / Dance / Speed / Games)

ROLLER INLINE SKATING (Quads or Roller Skates welcome), proposed by Nicolas CONSTANTIN and assistants.

Multidisciplinary activity that attracts the most fans and makes the happiness of young and old.

f) Free skating on musical background, these are all special features that will be presented to our participants.

SECURITY RULES:
Wearing protectives is mandatory.
The parking will be exclusively reserved for our activities.
In case of minor injuries, First Aid will be provided.
In case of more serious injuries thank you to communicate an emergency number and the person to contact.
A Disclaimer (see below) must be read, approved and duly signed by the legal guardian of the participant if minor or by the participant himself if major.

I hereby agree that the instructor/ staff is responsible for the mandatory supervision and is authorized to take the necessary measures to prevent any damage. In the event that participation is refused by the organizers before the start of the lesson, I agree to pay any total amount of potential costs that would result. Additional note:
I, the undersigned acting on my own behalf or in my capacity as parent or guardian of the minor detailed above, hereby declare that they have read the rules and all the conditions relating to 'ROLLER SKATING LESSONS' offered by Nicolas Constantin, legal representative of NICOSPORTS LTD, and undertakes both my name and that of the minor (s) mentioned on the Recto de this document for which I vouch to me to comply with the said rules and to respect all the said conditions they were communicated in writing or verbally. Notwithstanding the generality of the provisions contained in the foregoing paragraph, I, the undersigned, acting on my own behalf or in my capacity as parent or guardian of the aforementioned minor, hereby make a solemn commitment both personally and to me. the above-mentioned minor for whom I am vouching for more specific compliance with the following conditions.I will pay, within the time limits prescribed by Nicolas Constantin legal representative of NICOSPORTS LTD, any charges relating to the Initiation of Roller, Skateboarding, Cruising, Crossminton or any other activities. I also undertake to ensure the wearing of any equipment, whether protective or otherwise, that the said company will indicate and judge necessary for the initiation to the different sports and leisure activities offered by NICOSPORTS LTD. In the event of accidents resulting in injury or not, I hereby authorize the employees to take all the measures they deem necessary in the circumstances and further undertake not to hold them responsible, either at civil only criminal, for any provision made by them under the available of this paragraph. Finally, I declare and acknowledge that under no circumstances the responsibility Nicolas Constantin legal representative of NICOSPORTS LTD and its employees, employees, representatives, agents, assigns and attendants can not be sought for any damage or prejudice or because of my participation in any of the activities by me or the minor mentioned above or the Recto of this document.It is also expressly agreed that the activity "ROLLER SKATING LESSONS" is exclusively managed by Nicolas Constantin legal representative of NICOSPORTS LTD and that the latter is not in partnership with the owner of the premises where the transaction is located; the said owner being totally foreign to the convention that unites me to Nicolas Constantin hereby. I authorize my child to make photos either in group or individually but directly related to the present animation (press, television, web), however not for advertising purposes. I hereby agree that my son / daughter participates in the ROLLER SKATING LESSONS.
