BY SUBMITTING THIS APPLICATION, YOU ACKNOWLEDGE THAT YOU ARE THE PARENT OR STUDENT APPLYING.  ONCE YOUR PARKING PASS IS ISSUED, YOU MAY NOT ALLOW OTHER STUDENTS TO USE YOUR PARKING PASS.  IMPROPER USE OF YOUR PARKING PASS OR PARKING SPOT COULD RESULT IN LOSS OF PARKING PRIVILEGES AND/OR DISCIPLINARY ACTION.

Once all is complete and fees are paid, student must present photo or physical copy of insurance information, drivers license, vehicle registration and completed Parking Contract to receive parking hang tag.
