I may apply for a NSW Tertiary Student Concession (Concession) for travel on public transport in NSW.

By providing my consent below I acknowledge and understand that:


1. My eligibility for a NSW tertiary concession travel entitlement for travel on public transport in NSW will be assessed by my tertiary institution in accordance with guidelines published by Transport for NSW (TfNSW) pursuant to the Transport Administration Act 1988 (NSW) or other legislation (‘Guidelines’).
2. I consent to my name, student number, date of birth, the name of my tertiary institution, course expiry date (if applicable) and my assessed eligibility for concession travel (Personal Details) being disclosed by my tertiary institution to TfNSW, for the purpose of determining eligibility for and administering concessions and for directly related purposes such as audit, management and operational activities for concessions.
3. My Personal Details will be transferred to TfNSW in a secure file of all eligible students who consented to the disclosure in accordance with this notice.
4. My Personal Details may also be viewed by TfNSW as part of a non-site audit.
5. If I consent to the above disclosure of my Personal Details, I understand this information will be held by TfNSW of 18 Lee Street, Chippendale, NSW 2008 and that I can submit a request to TfNSW for access to or amendment of any of this information which is held by TfNSW.
6. If I do not consent to my tertiary institution disclosing my Personal Details, my eligibility for a concession will not be able to be validated by TfNSW and I will not be eligible for a NSW tertiary concession travel entitlement and will be unable to obtain a Transport Concession Entitlement Card or aConcession Opal card.
7. I can withdraw my consent at any time by notifying my tertiary institution.
8. If I withdraw my consent or cease to become eligible for a NSW tertiary concession travel entitlement, my tertiary institution is required to notify TfNSW. This will be done by removing my Personal Details from the file of eligible students provided to TfNSW.
9. I understand that TfNSW will withdraw concession benefits If and when I do not meet the eligibility criteria determined by TfNSW.

• NSW Tertiary Student Concessions are for the exclusive use of the nominated card holder and are not transferrable.

NSW Tertiary Student Concession Opal card Holders:
• The Concession Opal card is subject to the Opal Terms of Use available at transportnsw.info/opal-terms-of-use.
• The Concession Opal card along with proof of entitlement in the form of a Transport for NSW issued Transport Concession Entitlement Card or an Approved Form of Student ID Card must be produced while travelling on a concession fare if requested by a bus driver, Revenue Protection Officer or Police.
• If the Concession Opal card is lost, stolen or damaged, the cardholder must log into their Opal account at transportnsw.info and report their card as lost or stolen.

NSW Tertiary Student Transport Concession Entitlement Card Holders:
• The Transport Concession Entitlement Card is subject to the Terms and Conditions available at www.transportnsw.info/tertiary-students.
• Outside the Opal network the Transport for NSW issued Transport Concession Entitlement Card must be produced when purchasing a concession
ticket product, and while travelling on such a ticket, if requested by a bus driver, Revenue Protection Officer or Police.
• In the Opal network, while travelling using a Concession Opal card, either an Approved Form of Student ID Card or a Transport for NSW issued Transport Concession Entitlement Card must be carried at all times and produced upon request by a bus driver, Revenue Protection Officer or Police.
• If the Transport for NSW issued Transport Concession Entitlement Card is lost, stolen or damaged, it must be reported to Transport for NSW. An application for a replacement card should be submitted to Transport for NSW.
