If you have lost your student ID or your ID no longer scans please complete this form. Only the most current ID will work so if you locate your lost/missing ID after a replacement is issued you must throw the old one away. If you have any questions please contact the Main Office at 503-738-5586 / Si ha perdido su identificación de estudiante o su identificación ya no se escanea, complete este formulario.
