If you plan to use insurance for your appointments, or if you have one of the insurances we accept,
a signed referral from your primary care provider will be necessary. This can be any prescriber. Please download the referral form provided
below, have your provider complete and fax it to our office. For security reasons, please avoid emailing this form.
Additionally, while Holistic Community Therapy specializes in working with adults with mental health conditions and cognitive changes, we understand the importance of finding the right fit for your needs.
