This form is CSMOA's primary method of collecting & updating our membership database containing new and existing members.

Existing members will be asked annually to fill in this form (around the time of membership payments) so information held within the database is kept up to date.

All the data collected in this form is securely stored and managed under CSMOA's membership information DPIA document (Data Protection Impact Assessment), relevant Privacy Notice, and Privacy Policy, which have been designed under GDPR regulations and are both reviewed regularly. The general CSMOA Privacy Policy is available on our website, and the specific Privacy Notice for membership data can also be viewed via the CSMOA website (www.csmoa.org.uk/privacy). The Privacy Policy details how individuals can exercise their rights provided under GDPR.

CSMOA will primarily use email as its main form of communication (directly from our @csmoa.org.uk emails, or via WhoseTheUmpire (WTU) our match administration software.
