Club Proposal Form
Please fill out this form if you are interested in beginning a school-sponsored club. After the completed form has been reviewed, a meeting will occur between the potential sponsor and/or students and the school counselor. The counselor will then collaborate with school administration and a decision to either accept or reject the club will be communicated to the sponsor and/or students. Please note: if a club is accepted as a school-sponsored club, the club will be on "pilot" status for two full semesters.
