OPTION 1: CO-APPLICANT IN THE APPLICATION

Whether your church or organisation would be willing to be cited as a co-applicant in the application, and if yes kindly answer YES to OPTION 1 below and complete the name of your organisation, the number of congregants etc. (Note that, as we cannot list everybody as co-applicants, the legal team will select who will be the best co-applicants for the case, so as to also be as broadly representative as possible).
NOTE: In order to be considered, we require a copy of the constitution of your organisation in addition to the other information requested sent to michael.swain@forsa.org.za.

OPTION 2: FOR SA MANDATE TO ACT ON YOUR BEHALF

Whether your church or organisation would be willing to give FOR SA a mandate to act for and on your behalf – and if yes, kindly answer YES to OPTION 2 and complete the name of your organisation, the number of congregants etc.. (The number of churches and groups who could be represented by FOR SA in this way, is unlimited).
