This form is going to ask you to provide your contact information (Nominator) and details of the person's or group's (Nominee) work beyond the scope of their everyday job/position. Former winners from the past 3 years are not eligible to win again (see below).  

Categories for Nominations are - One (1) Undergraduate Student, One (1) Graduate Student, One (1) Student Club/Organization, One (1) Staff Member, One (1) Alumni and One (1) Faculty Member

The deadline for submissions is Friday, December 16 by 5pm.
