High School Student Device Agreement Form
This form is a backup to the online registration process.  NSHS will be using it to move the device checkout process at an improved speed.  Students opened the form with their @students.fwisd.org gmail and will need to use their parental email for electronic signature on the rest of the form.  Students cannot sign their own form and receive a device.

Repairs will be different for Chromebooks.  Repair assistance will be available in the library during 5th period lunches.  No class time may be lost for repairs so plan ahead.  The limit is 1 repair per device (according to warranty) so please use care when handling your device or you may have replacement costs. Refer to attached documents for costs/fees associated with repairs for the 2019-2020 school year.

Hotspots will be available to each student this year.  They were acquired through a grant process which means they are free to use, but carry stiff penalties if lost ( as they don't belong to FWISD). Please indicate on this form if you wish to be granted a hotspot when they arrive this year.
