NOTE: This form is for use by fully online Tiger Virtual Academy students AND Hybrid/Remote cohort students when in an offsite remote learning mode.  When in their buildings for onsite learning mode, Hybrid/Remote cohort students must use established in-person Chromebook repair procedures.

Please follow the following procedure to get your student Chromebook repaired.
